Enviromental Health Services are located on the second floor of the Main Health Department. Office hours are 8 a.m. – 4 p.m. The office is closed from noon – 12:30 p.m. for lunch.Â
Applications are accepted at the Environmental Health Services office either by hand delivery, mail, fax (813-272-7242), or email.
Applications sent by fax or email will require a Credit Card Authorization Form to be completed or you can telephone the credit card information to our office for payment.
