Our Environmental Health program is located on the second floor.
Open from 8 a.m. – 4 p.m. Monday – Friday
Closed daily from noon – 12:30 p.m.
Applications and Forms
You can submit your application through email, mail, or fax it to 813-272-7242.
You can also submit your application in person at our main office.
Applications sent by fax or email will require a completed credit card authorization form, or you can pay over the phone by calling 813-307-8059.
