The Florida Department of Health in Hillsborough County’s (DOH-Hillsborough) Immunization Program promotes and protects the public’s health through reduction and eventual elimination of vaccine-preventable diseases by providing vaccine administration, education on receiving timely immunizations, surveillance, and monitoring. We provide immunizations for children and adults. Adult immunizations are offered on a fee for service basis. Eligible children receive no cost vaccines through the Vaccines for Children Program.

The Immunization Program does not provide walk-in services. Call 813-307-8077 to schedule your appointment.

  • Please bring your picture ID and all previous immunization records for each individual needing services to your appointment
  • Prior records must be available prior to or at visit time

Immunization services are located at our Sulphur Springs Office from 7:30 a.m. – 4 p.m. On the second and fourth Tuesday of each month, we offer morning services only from 8-11 a.m.

Travel Immunizations

The Florida Department of Health in Hillsborough County (DOH-Hillsborough) no longer offers international travel vaccinations. The Florida Department of Health in Manatee,  Polk, and Pinellas Counties offer Travel Immunization Programs.

Vaccines for Children Program (VFC)

The Vaccines for Children Program offers vaccines at no cost for eligible children. Children through 18 years of age who meet at least one of the following criteria are eligible to receive VFC vaccine:

  • Medicaid-eligible
  • Uninsured
  • American Indian or Alaska native
  • Underinsured

Children whose health insurance covers the cost of vaccinations are not eligible for VFC vaccines. Children enrolled in the Florida KidCare Program, including MediKids and the Healthy Kids program, are insured and therefore not eligible to receive VFC vaccines.

If your child has a pediatrician that provides immunizations, it is recommended that you take your child to their pediatrician for immunizations.

Documents

Parents are required to bring their photo identification and child’s immunization records. Non-custodial parent or guardian must bring documentation of consent for treatment in addition to photo identification.

Religious Exemptions (DH Form 681)

Individuals requesting Religious Exemptions for children need to apply in person and bring child birth certificate and parent/guardian ID. Religious Exemptions do not require an appointment.

The Florida Department of Health (DOH) does not provide Religious Exemptions for adults or persons who have graduated 12th grade.

College Forms

The DOH will complete college forms if all documentation is provided at the time of visit (Photo ID and Immunization History). A fee of $25 is collected for the completion of college forms. This service is only available by appointment.
