The Florida Department of Health in Hillsborough County conducts a Community Health Assessment (CHA) and creates Community Health Improvement Plan (CHIP) every five years. These efforts help us understand the current health status of our community, identify key challenges and opportunities, and guide collaborative strategies that improve health and well‑being across the county.


Healthy Hillsborough Collaborative

The Healthy Hillsborough Collaborative (HHC) is a countywide network of community partners, organizations, and residents working together to improve health and well‑being across Hillsborough County. The Collaborative brings people from diverse sectors to a shared table to identify community needs, align efforts, and support the CHA and CHIP.


CHIP PRIORITY AREAS

The 2025 Priority Areas identified by the Healthy Hillsborough Collaborative are:

  • Behavioral Health 
  • Built and Natural Environment 
  • Social and Economic Factors

Each priority area is supported by a standing committee that meets regularly to advance action planning, resource sharing, and collaborative problem‑solving.
