Tattoo Artist Licensure

Environmental Health

Tattoo Artist Licensure is required for:

Conventional Tattooing
  • Persons who tattoo the body of another human.
  • Persons who practice cosmetic tattooing, permanent make-up, microblading, or micropigmentation.
  • Students/apprentices who tattoo other humans as part of their tattoo training (ex. live models).
  • Tattoo or permanent makeup artist licensed outside of Florida.

A Tattoo Artist License requires submission of the following to the local County Health Department having jurisdiction of the tattooing program where you live.

Initial Tattoo Application Requirements:

  • Fully completed Application for Tattoo Artist License (check initial on application).
    • The Application must be completed with the name of the licensed tattoo establishment where the tattoo artist will perform tattooing or intends to perform tattooing. Applications will not be accepted with this field left blank or written in as N/A.

  • $60 application fee
  • At least 18 years of age
  • Copy of government-issued photo ID (ex. Driver’s license, passport, green card, military ID)
  • Successfully completed a Florida bloodborne pathogen and communicable diseases course with a minimum score of 70%. (Submit certificate, application and all required documents to DLCHD29Tattoo@flhealth.gov)

Tattoo Artist Renewal Application Requirements:

To renew a tattoo artist license please submit the following to the local county health department where you reside.

  • Fully completed Application for Tattoo Artist License (check Renewal on application).
    • The Application must be completed with the name of the licensed tattoo establishment where the tattoo artist will perform tattooing or intends to perform tattooing. Applications will not be accepted with this field left blank or written in as N/A.
  • Application must be fully completed and submitted annually to receive current tattoo artist license
  • $60 Renewal fee
  • Submit all required documents to DLCHD29Tattoo@flhealth.gov

Reactivate an Expired Tattoo Artist License:

To reactivate a an expired tattoo artist license, submit the following to the local county health department where you reside:
  • Fully completed Application for Tattoo Artist License (check Renewal on application).
    • The Application must be completed with the name of the licensed tattoo establishment where the tattoo artist will perform tattooing or intends to perform tattooing. Applications will not be accepted with this field left blank or written in as N/A
  • $85 Reactivation fee

Guest Tattoo Artist Registration in Florida

  • Guest registration is valid up to 14 consecutive days in Florida
  • A guest tattoo artist registration is required for any tattoo artist who plans to tattoo in Florida and who also holds an active tattoo artist license, registration, or certification issued by a jurisdiction outside of Florida

If your state does not require a tattoo artist license registration or certification, you must apply for a Florida Tattoo Artist License.

Guest Tattoo Artist Registration Requirements

  • Fully completed Application for Guest Tattoo Artist Registration.
    • The Application must be completed with the name or names of the licensed tattoo establishment or event of where the tattoo artist will perform tattooing or intends to perform tattooing. Applications will not be accepted with the establishment field left blank or written as N/A.

  • $35 Registration Fee
  • Be at least 18 years of age
  • Copy of government-issued photo ID (ex. Driver’s license, passport, green card, military ID)
  • Proof of an active, current license, registration or certification outside of Florida for tattooing
  • Documentation of completion of a Florida bloodborne pathogen and communicable diseases course with a minimum score of 70%. (Submit certificate, application and all required documents to DLCHD29Tattoo@flhealth.gov)

Submit application and required documents at least 14 days prior to initial day of licensure indicated on application to allow for processing.


**Review the Guest Tattoo Artist Registration Guide to ensure Florida will recognize your state’s tattoo artist license.

** Submission of the wrong application will delay your licensure.  In most cases, it’s better to pay for an annual Florida Tattoo Artist License for $60.00 than pay for multiple guest Artist Registrations at $35.00 each.
