HOW TO PAY AND SUBMIT APPLICATIONS FOR TATTOO LICENSURE
Permit fees and applications for initial and renewal applications can be submitted by mail to:Department of Health in Hillsborough County

*Please do not mail cash. Acceptable methods of payment for mail are money order, check, cashier’s check, and credit card authorization.
- Completed applications can be faxed to: 813-272-7242
- Payments can be made over the phone to 813-807-8059 with Visa and Mastercard only. Please provide your name and permit number(s).
- Renewal payments can be made at myfloridaehpermit.com with Mastercard, American Express, E-Check, and Discover Card. Visa is not accepted.
*Online applications are not accepted through this payment portal and must be faxed, mailed, e-mailed to a clerk or tattoo coordinator directly, or dropped off in-person. Call 813-307-8059 for direct e-mails/contact information.
- Applications can be completed, existing permits renewed, and payments processed at the main DOH Hillsborough office with Visa, Mastercard, cash, check, money order, and/or cashier’s check.
