Office hours are 8:00 a.m. – 4:00 p.m (we are closed for lunch from 12:00 p.m. – 12:30 p.m.)
Applications are accepted at the Environmental Health Services office either by hand delivery, mail, fax (813) 272-7242 or e-mail.
Applications sent by fax (813) 272-7242 or e-mail will require a Credit Card Authorization Form to be completed or you can telephone the credit card information to our office for payment.
