If you are sick from a food you ate, file a food-borne illness complaint.
If your complaint is about an institutional facility (Assisted Living Facility (ALF), Bars/Lounges, Detention Facilities, Fraternal Organizations, Private/Public Schools, etc.) that are licensed by the Department of Health click here to file a complaint.
If your complaint is about a restaurant, file a Restaurant Complaint with the Department of Business and Professional Regulation (DBPR).
If your complaint is about a grocery stores, convenience stores, meat market, coffee shops, or ice and water vending machines, file a complaint with the Department of Agriculture and Consumer Services (DACS).
