MapRELIGIOUS EXEMPTIONS (DH Form 681) - Individuals requesting Religious Exemptions for children need to apply in person and bring child birth certificate and parent/guardian ID.
Religious Exemptions do not require an appointment.
The Florida Department of Health does not provide Religious Exemptions for adults or persons who have graduated 12th grade.
