Religious Exemption Form - DH 681

Immunization Services

RELIGIOUS EXEMPTIONS (DH Form 681) - Individuals requesting Religious Exemptions for children need to apply in person and bring the child's birth certificate and parent/guardian's ID. 

Religious Exemptions do not require an appointment. 

The Florida Department of Health does not provide Religious Exemptions for adults or persons who have graduated 12th grade.
