Department of Health Permits for School Facilities

Public/Private/Charter School Facility

 

Private and Charter Schools require a health department Group Care and Food Sanitation Permit.   Public Schools require a Food Sanitation Permit only. 

Submit the following for a Group Care Permit:

1.  School Plant Application

2.  Facility Floor Plan

3.  Outside Site Plan

4.  Application Fee (see application)

All schools providing food service are required to have a health department Food Sanitation Permit.  This permit is an annual requirement.

Submit the following far a Food Sanitation Permit:

1.  Plan Review Application

2.  Plan Review Checklist

3.  Full Kitchen Plan

4.  Plan Review Fee ($40.00)

5.  Food Sanitation Permit Application  (After the Plan Review has been approved)
