Community Health Assessment and Improvement Planning

Florida Department of Health - Hillsborough County

CHA/CHIP and the MAPP Model

The Florida Department of Health in Hillsborough County conducts a Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) every five years to determine the current health status of the community and address the key health-related challenges and opportunities in Hillsborough County. In the 2024-2025 cycle, the Department is using the Mobilizing for Action through Planning and Partnerships (MAPP 2.0) planning model developed by the National Association of County and City Health Officials (NACCHO) to engage and collaborate with community members and organizations. This model uses three tools to collect and analyze quantitative and qualitative data which are the Community Partner Assessment (CPA), the Community Status Assessment (CSA), and the Community Context Assessment (CCA). The results from these assessments are used to determine priority areas for the County and develop and implement a CHIP with initiatives and strategies to improve the community's health over the next five years.

Your information is confidential, and the surveys will be available from March to April 2024 in English, Haitian Creole and Spanish. Healthy Hillsborough Collaborative

The MAPP 2.0 Process focuses on engaging with multiple people, groups, and organizations to gain a unique perspective on the social and economic conditions impacting health, where we live, work, learn, play, and age. This is a community-driven model led by a Steering Committee of partners, under the name Healthy Hillsborough.

Healthy Hillsborough was formed in October 2015 as a collaboration between DOH–Hillsborough, Florida Hospital, Moffitt Cancer Center, St. Joseph's Hospitals and South Florida Baptist Hospital, Shriner’s Hospital for Children-Tampa, Suncoast Community Health Centers, Tampa Family Health Centers, and Tampa General Hospital. Healthy Hillsborough was established to complete a comprehensive Community Health Assessment (CHA)/Community Health Needs Assessment (CHNA) and to identify opportunities for collaboration to collectively impact and improve the health of Hillsborough County. In 2023, the Steering Committee membership expanded to include local community-based organizations working across many sectors of public health, including: early learning, parks & recreation, migrant services, housing, Hispanic services, domestic violence, transportation, homelessness, food security and others. Healthy Hillsborough also includes a broader collaborative of community partners who have helped to determine the top health priorities based on primary and secondary data collected in our community, and who have participated in the development of the current Community Health Improvement Plan.

CHIP Priority Areas

The three Priority Areas identified in 2019 by Healthy Hillsborough for the County are behavioral health, access to health services, and exercise, nutrition, and weight.

The All4HealthFL collaborative, formed in 2018, is collectively addressing behavioral health priorities. The remaining areas are being addressed through the implementation plans described in the CHIP, developed by the Florida Department of Health in Hillsborough County and members of the Healthy Hillsborough Collaborative.
